Medical Examiner's Evaluation Form

Dear Undercurrent,

Regarding all your articles lately about senior divers, DAN offers a form that MDs can fill out stating that the named person is medically cleared for diving. DAN has worked with the Hyperbaric Medicine Division at UC San Diego to create a form for the doctor to fill out as part of the participant's medical questionnaire. Are you aware if this is accepted by dive shops/resorts if they have questions about the ability of a senior to dive?

Dive Centers set their own rules, but most accept a letter from a doctor, so a DAN form signed by a doctor would be even better received. If you have concerns, we recommend completing the form and emailing it to the dive center before you pay to ensure they'll approve you for diving. You can download the form here:
